Special Instructions:
Please ensure that all waste is double bagged in red biohazard bags and that those are contained in a leak proof, puncture proof container (Stericycle box).
Indicate above if your non-preserved animal tissue requires refrigeration until disposal vendor comes to pickup all materials for disposal (2-4 times per calendar year). Cold storage space is
limited and on a first come, first served basis.
For preserved animal tissue, no surplus liquid within the tissues and no additional non-tissue materials (glass, sharps, gloves, paper towels etc.) are present.
Sharps should only be packaged with other sharps.
PIMW/Human Medical Waste gowns, masks, gloves, and other contaminated PPE should only be packaged with other contaminated PPE.
NOTE: For "Type of Contents" below, please indicate one of the following for each container: Preserved Animal Tissue, Non-Preserved Animal Tissue, Medical Waste/PIMW or Sharps.
